KPK Regional and District Health Authorities Act 2019:

Gradual Privatization of Government Hospitals Will Crush the Suffering Poor

While passing the Regional and District Health Authorities Act 2019, the KPK Provincial Minister of Health, Dr. Hisham Inam Ullah Khan, declared that, “we are a poor country, we don’t have enough capacity that we can provide free health service to everyone, provide free medicine and free doctors. We will bring public-private partnership in some places and raise costs as well in some places.” The regime that claims to make Pakistan a state like Madina, providing welfare for its people, is exposed. Clearly, Democracy is designed only to benefit the capitalist, ruling elite, providing legal cover to their corruption, whilst leaving the poor to face the brutality of capitalism alone. After the passage of this new act, all provincial hospitals, rural health centers, dispensaries, mother and child care centers and government laboratories will be handed over to regional and district health authorities. These authorities will receive limited financial resources and the rest have to be arranged by themselves, which simply means that either the poor will pay to cover their state hospital medical bills or they will have to simply die off in anguish.

Already 75% of the population of Pakistan gets health treatment from private hospitals, because state hospitals not only lack facilities, their quality of care is abysmal. Now even the poor, which have no choice but to use state hospitals is deprived of free or low cost care, as bad as it is. Democracy has made basic health services a luxury, where patient are considered paying customers, to be squeezed to ensure hefty profits. Despite this stark reality, the current rulers insist they are not privatizing healthcare and only improving state hospitals. In the capitalist West, people are facing the same issues with health care, as health facilities are handed over to insurance companies. Globally, Democracy has ensured that capitalist insurance companies and private hospitals to secure substantial profits.

Islam declares health, education and security societal needs. Islam obliges the Khaleefah to ensure the availability of such facilities in society. Imam Muslim narrated from Jabir (R.A),

رَسُولُ اللهِ صَلَّى اللَّهُ عَلَيْهِ وَسَلَّمَ إِلَى أُبَيِّ بْنِ كَعْبٍ طَبِيبًا، فَقَطَعَ مِنْهُ عِرْقًا ، ثُمَّ كَوَاهُ عَلَيْهِ

“Jabir reported that Allah’s Messenger (ﷺ) sent a physician to Ubayy b. Ka’b. He cut the vein and then cauterised it.”

Al-Hakim narrated in his Mustadrik from Zaid Bin Aslam, who narrated from his father that he said: “I fell severely ill in the era of Umer Bin Khattab, so he called a doctor who heated by body and after that I became so strong that I used to chew the date seed.” And RasulAllah (saaw) said,

وَالإِمَامُ رَاعٍ وَمَسْئُولٌ عَنْ رَعِيَّتِهِ

“The ruler is a guardian and responsible for his subjects” (Al-Bukhari: 2751).

Providing health facilities in society are one of the basic responsibilities of an Islamic State. Khaleefah Al-Mansor established a hospital, Bimaristan Al-Mansori, in 1283, and until today, the world has not witnessed such a great hospital. It could accommodate eight thousand patients, in which every patient was provided two attendants to take care of him. Democracies and dictatorships have failed to protect the rights of the people. Only the Khilafah (Caliphate) on the Method of Prophethood, ruling by all that Allah (swt) has revealed, can secure humanity from the capitalist system of looters and plunderers.

سرکاری ہسپتالوں کی درجہ بدرجہ نج کاری غریبوں کی جانوں سے کھلواڑ ہے

خیبر پختونخواہ کے وزیر صحت ڈاکٹر انعام ہشام اللہ نے صوبائی اسمبلی میں ڈسٹرکٹ ہیلتھ اینڈ ریجنل اتھاریٹیز ایکٹ 2019 بل پاس کرتے ہوئے پشتو میں تقریر کرتے ہوئے کہا، ”ایک بات مجھے اور آپ کو تسلیم کرنی ہوگی، ہم ایک غریب ملک ہیں، ہماری اتنی capacity نہیں ہے، کہ ہم ہر ایک کو مفت علاج مہیا کریں، مفت دوائیاں دیں، فری ڈاکٹر دیں۔ تو کہیں نہ کہیں ہم پبلک پرائیویٹ پارٹنر شپ بھی لائیں گے، اور کہیں نہ کہیں ہم ریٹس بھی بڑھائیں گے۔“ پس ریاستِ مدینہ اور سوشل ویلفئیر کے دعوے کرنے والی اس حکومت نے برسربازار اپنا بھانڈا پھوڑتے ہوئے جمہوریت کی حقیقت بھی عوام پر ننگی کر دی کہ یہ صرف سرمایہ داروں اور امراء کی آمریت اور قانونی لوٹ مار کا نظام ہے۔ جبکہ یہ نظام غریب کو سرمایہ داری نظام کے گرم تھپیڑوں کے حوالے کر دیتا ہے۔

اس نئے ایکٹ کے پاس ہونے سے تمام صوبائی اسپتال، دیہی مراکزِ صحت، ڈسپنسریاں، زچہ و بچہ مراکز اور سرکاری لیبارٹریاں ریجنل اور ڈسٹرکٹ ہیلتھ اتھاریٹیز کو منتقل ہو جائیں گی، جن کو محدود گرانٹس دی جائیں گی، جبکہ باقی اخراجات کا انتظام ان کو اپنے پلے سے کرنا ہوگا، اس کا مطلب ہے کہ غریب عوام یا تو اپنا خون نچوڑ کر سرکاری ہسپتالوں کے بل بھریں، یا ایڑیاں رگڑ رگڑ کر مر جائیں۔ پہلے ہی پاکستان کی 75 فیصد عوام اپنا علاج پرائیویٹ ہسپتالوں اور کلینکوں سے کراتی ہے کیونکہ سرکاری ہسپتالوں کی سہولیات نہ صرف یہ کہ ناکافی ہیں بلکہ ان کا معیار بھی انتہائی گھٹیا ہے، اس پر مستزاد، وہ غریب طبقہ جو مجبوراً سرکاری اسپتالوں کا رخ کرتا تھا، ان کو بھی اس سے محروم کیا جا رہا ہے۔ یعنی جمہوریت نے بنیادی صحت کو بھی ایک عیاشی بنا کر رکھ دیا ہے، جہاں مریض کو مریض نہیں بلکہ گاہک سمجھا جاتا ہے جس سے ہوشربا منافع کمانا مقصود ہو۔ اس پر یہ حکمران ڈھٹائی سے یہ دعویٰ بھی کر رہے ہیں کہ ہم اسپتالوں کو بہتر کر رہے ہیں اور یہ کہ کسی قسم کی کوئی نجکاری نہیں کی جا رہی۔ یہی حال مغرب کا ہے جہاں صحت کی سہولیات انشورنس کمپنیوں کے حوالے کر کے حکومت اس بنیادی معاشرتی ضرورت کو بھی سرمایہ دار انشورنس کمپنیوں اور پرائیویٹ اسپتالوں کے بے پناہ منافعوں میں تبدیل کر چکی ہے۔

اسلام صحت، تعلیم اور سیکیوریٹی کو معاشرے کی بنیادی ضرورت گردانتے ہوئے خلیفہ کو اس بات کا پابند بناتا ہے کہ وہ معاشرے میں ان سہولیات کی دستیابی کو یقینی بنائے۔ امام مسلم نے جابرؓ سے روایت کیا:

”رَسُولُ اللهِ صَلَّى اللَّهُ عَلَيْهِ وَسَلَّمَ إِلَى أُبَيِّ بْنِ كَعْبٍ طَبِيبًا، فَقَطَعَ مِنْهُ عِرْقًا ، ثُمَّ كَوَاهُ عَلَيْهِ

”رسول اللہ ﷺ نے ایک طبیب کو ابی بن کعبؓ کے پاس بھیجا جس نے نس کاٹ کر اسے داغا۔“

الحاکم نے مستدرک میں زید بن اسلم سے، جنہوں نے اپنے والد سے روایت کیا کہ انھوں نے فرمایا: “میں عمر بن الخطابؓ کے دور میں شدید بیمار ہوا، تو انھوں نے ایک طبیب بلایا جس نے میرے جسم کو اتنی گرمی پہنچائی کہ میں کھجور کی ہڈیاں چوسنے لگا۔” اور رسول اللہ ﷺ نے ارشاد فرمایا:

والإمام راع ومسئول عن رعيته

”امام نگہبان ہے اور اپنے رعایا کیلئے ذمہ دار ہے۔“(البخاری: 2751)

یہ دلائل واضح کرتے ہیں کہ صحت کی سہولیات معاشرے کو بہم پہنچانا ریاست کی بنیادی ذمہ داریوں کا حصہ ہے۔ خلیفہ منصور کا بیمارستان المنصوری جو 1283ء میں بنا، آج بھی دنیا میں اس سے بڑا اسپتال نہیں بن سکا۔ اس میں 8 ہزار مریضوں کو داخل کرنے کی گنجائش تھی جس میں ہر مریض کیلئے دو اٹینڈنٹ تھے۔ جمہوریتیں اور آمریتیں عوام کے حقوق کا تحفظ کرنے میں ناکام ہیں، صرف وحی الہی کے ناقابل تبدیل اصولوں کی بنیاد پر بنے خلافت کا نظام ہی انسانیت کو ان ماڈرن ڈاکوؤں کے نظام سے نجات دلا سکتا ہے۔
